Required Information
±«Óãtv Student Health Services reviews your health information and verifies immunization requirements through the confidential wellness portal.
±«Óãtv requires the completion of the following:

- Submission of TB testing (if required) based on your answers to the TB Screening Questionnaire
You will find these requirements and instructions on submitting the required health information at the ±«Óãtv new student checklist
Students who lack adequate immunizations and/or do not submit immunization information will be unable to check in and receive their room key upon arrival. We have a limited supply of common medications prescribed by a clinician at a minimal cost when a student is seen at SHS. - Emergency contact(s) telephone numbers
- Bring the contact information for your current health care providers
